BridgeMakers: Community Health Leader Training is an MDHHS-approved program designed to prepare Community Health Workers (CHWs) and Community Health Representatives (CHRs) to serve as trusted links between communities and health systems. This training equips participants with the knowledge, practical tools, and real-world experience needed to improve access to care, address social determinants of health, and support individuals across diverse community settings.

This program combines structured instruction with applied learning to ensure participants are fully prepared for real-world CHW roles.
